Please join us at our monthly support group "Talk Time". This is your time to meet other EFMP families, network, make new friends, learn about available resources, share ideas and information, and to just take a little time to relax! This is also a great time to give your kids some playtime as we welcome children. Light refreshments are served. All branches of the military are welcome.

WHAT IS THE EXCEPTIONAL FAMILY MEMBER PROGRAM
The Exceptional Family Member Program (EFMP) is a mandatory program per Marine Corps Order P1754.4A. The purpose of the program is to assist monitors at HQMC in assigning Marines to locations where the needs of their EFM can be met. The program also assists service members by providing support and case management before, during and after relocation required by a change of duty station.
WHO QUALIFIES FOR EFMP
An Exceptional Family Member (EFM) can be a spouse, child, stepchild, adopted child, foster child or dependent parent who meets the following criteria:
Possesses a physical, intellectual or emotional disability and requires special medical and/or educational services.
Enrolled in DEERS
Resides with the sponsor (exceptions include geographical bachelors and family members receiving inpatient care or living in an educational setting.)
HOW DO I ENROLL
• Ensure your family member is enrolled in DEERS.
• Contact the EFMP office for assistance and information.
• Complete the DD Forms and return to the EFMP office for processing. Forms may be downloaded from the links below.
The DD Form 2792 Medical Summary must be completed for ALL enrollees even if the EFM is a child who is being enrolled for educational reasons. The medical summary portion (pages 3-5) must be fully completed and signed by each attending medical provider. (Must be MD, Physician’s Asst. or Family Nurse Practitioner.) The DD Form 2792 must be signed on pages 1 2 by parent/guardian or by EFM of majority age.
ADDENDUM 1
Asthma/Reactive Airway Disease Summary This section is to be completed and signed by a medical provider if the family member has a history of asthma or reactive airway disease.
ADDENDUM 2
Mental Health Summary This section is to be completed by the mental health provider if the family member has a history associated with mental health.
This form must be completed and signed by a school official for ALL children in school whether or not they are enrolled in a special education program. For children 3 and under this form only needs to be completed if they have an Individual Family Service Plan (IFSP). A copy of the current Individualized Education Program [ IEP ] or IFSP must be submitted.
BENEFITS OF ENROLLMENT IN EFMP
Family support/case management - Informed assignment screening - 40 hours of respite per month per family - Special housing consideration (determined by HQMC) - Continuation on location for Marines who qualify - EFMP attorney assistance - Assistance with TriCare ECHO (Extended Care Health Options) enrollment - Relocation assistance - Resources, referrals and linkages
For the San Diego area - Support at IEP meeting and other agency meetings - Support groups (Talk Time), events, potlucks, focus groups, workshops, trainings.
UPDATES AND DISENROLLMENT
It is the responsibility of the sponsor to update every two years or sooner if criteria for enrollment changes of if there is a change in family status. Failure to update does not disenroll you from the EFM Program. To disenroll the DD Form 2792 must be signed by EFM’s physician annotating that enrollment is no longer needed. For a change in family status such as divorce or death, please contact the EFMP office for guidance.
RESPITE CARE PROGRAM
If you are an EFMP enrolled Marine or Sailor working aboard MCRD or on recruiting duty in the Western Recruiting Region, you may be eligible for the Respite Care program. Respite care is a service that provides short-term or temporary care of a family member with special needs so that the regular caregiver may have some time off. This program allows a family enrolled in EFMP to receive reimbursement for up to 40 hours a month of respite care including coverage for siblings of children enrolled in EFMP. The rate of reimbursement is based on the special needs of the EFM and number of children in the family. Respite care delivery options may include a care provider who the parent selects, child care centers, special needs childcare centers, visiting nurse services or camps. Respite cannot be used to cover work or school related child care expenses. Please call the EFMP office for more information.
